How to file a claim for your damaged phone

Ultracom Wireless recommends Device Protection because Device Protection is a valuable, money-saving option for customers.  On most customers’ phones, the cost of the program in the first 22 months is less than the cost of a replacement phone.  Did you know that U.S. Cellular processes between 15,000 and 18,000 claims per month?  It costs less than .20 cents a day to add Device Protection.


  1. Cost:

    $6.99 per month for each basic phone, modem, Mobile Hotspot, Messaging Plus, and Home Phone.

    $8.99 per month for each Smartphone and tablet.

    How do I get Device Protection+ on my device?
    It’s easy to get Device Protection+ added to your account. Just stop into an UltraCom location or a US Cellular store and have it added!

    What are the deductibles?
    Once your claim is approved, nonrefundable deductible of $50, $150 or $175 will apply based on your device. Deductibles can be paid by credit card, debit card, eCheck or Cash on Delivery. A $17 charge will apply if you chose to pay by Cash on Delivery. Visit mywirelessclaim.com to determine which deductible applies to your device.

    Something Extra for Smartphones and Tablets…
    The following are additional protection features that can be downloaded and use for Smartphones and Tablet users:

    • Data Back Up: Back up and restore up to 2GB of your contacts, call logs, photos and videos so you don’t have to worry about losing all of the information on your device.
    • Locate, Lock and Wipe: Issue commands to locate, lock or wipe your device if it becomes lost or stolen. You can also sound an alarm, even if the device is in silent or vibrate mode.
    • Anti-Malware: Protect your device from hackers, malware and other threats using business-grade security software.

    How do I file a Claim and what information do I need?
    There are two ways to file a claim:

    • Visit mywirelessclaim.com 7 days a week, 24 hours a day to file a claim online.
    • Call 1-800-480-0167 to talk to a customer service associate.

    You will be provided with detailed information on how to return the device if applicable. The claim must be filed within 90 days of the incident, unless otherwise required by state law.
    Please have the following information available when you file the claim:

    • Your US Cellular wireless number
    • Device manufacturer/make/model
    • Cause and date of loss or damage
    • Deductible payment method

    Under certain circumstances, additional documentation such as a police report or proof of loss may be required in order to process the claim.
    Once your claim is approved, your device will be replaced with a reconditioned model of like kind and quality. In the event that a reconditioned one is not available, the device will be replaced with a new model of like kind and quality.
